STEPS TO CLAIMING CREDENTIALS THE FIRST TIME

CONGRATULATIONS!
You have completed both Year 1 and Year 2 of the FMA program, and are now ready to receive your ministers credential certificate and card.

1. Please fill out the form below and submit to [email protected]

2. Submit a PDF or Scanned Pastor Recommendation Letter to [email protected] or by uploading below.

3. Pay $35 Background Fee

PAY BACKGROUND FEE

4. Pay credential fee ($125 plus $5 for credit card processing)

5. You should receive your credentials in the mail within 7-10 Business Days

STEPS TO RENEW ACTIVE CREDENTIALS

FMA license and ordinations are renewed on an annual bases. Renewing your credentials is important. To ensure that your credentials are always in good standing. You can renew below.

1. Fill out and email FMA the annual questionnaire below. This allows us to see how you are using your credentials and also how we can further support you.
