RENEW YOUR CREDENTIALS

FMA license and ordinations are renewed on an annual bases. Renewing your credentials is important to ensure that your credentials are always in good standing.

STEPS TO ANNUALLY RENEW ACTIVE CREDENTIALS

1. Fill out and email FMA the annual questionnaire below. This allows us to see how you are using your credentials and also how we can further support you.

2. Pay annual credential fee. ($130 for individual or $210 for Married couples)
